For Grades 7-12, IDENTITY YOUTH CAMP revolves around the idea
of discovering who we are in Jesus. In Christ, we are loved infinitely, forgiven
completely, made brand new, and given life eternal in Him. In Jesus, we are alive.
The goal of this camp is to show these students that this is who they are
in Christ and why that matters right now.

We occasionally take photos and videos of Discover Camp events for use on our website, social media, and promotional materials.

Medical and First-Aid Information

ALL MEDICINES are to be given to the Camp Director at registration.
DO NOT send aspirin, cold remedies, or first-aid supplies. If it is necessary to send prescribed medication with the camper, please send in original container, clearly marked, and sealed in a zipper sealed bag with the child's name, what the medication is for, and instructions for taking medication (dosage/time).

Authorization for Treatment

All the above information is correct, and I give permission to the medical personnel, selected by the camp, to order x-ray, routine tests, treatment and necessary transportation for my child. In the event I cannot be reached in an emergency, I give permission to the physician selected by the camp to administer treatment, hospitalize, secure proper treatment for, and to order injection, anesthesia, or surgery for my child as named above.

Camper is not registered until this form is completed and submitted. Camper will not be registered without parent/guardian signature or if insufficient information is provided.
